Andrological Disorders and Sexual Dysfunctions

Male sexual health is a complex mechanism in which vascular structure, the nervous system, hormones, and psychological well-being all work together. Disruptions at any stage of this mechanism can negatively impact a person’s self-confidence and relationships with partners. Male Sexual Dysfunction and Its Treatment

  Erectile Dysfunction

It is the inability to achieve or maintain an erection sufficient for satisfactory sexual performance. It is not solely psychological; it can also develop due to conditions such as diabetes, hypertension, vascular blockages, or neurological disorders (organic causes). Following diagnosis, treatment plans may include oral medications, injections, shockwave therapy (ESWT), or, in necessary cases, penile prosthesis (penile implant) surgery.

  Premature Ejaculation

It refers to ejaculation occurring earlier than desired and with minimal stimulation during sexual intercourse. It is the most common sexual dysfunction in the general population. Once underlying prostate infections or hormonal imbalances have been ruled out, it can be successfully managed through behavioral techniques, topical anesthetic creams, oral medications, and psychological support.

  Testosterone Deficiency (Hypogonadism)

This condition occurs when the testicles are unable to produce sufficient levels of the male hormone (testosterone). It can develop as a result of advanced age (andropause), genetic factors, or testicular trauma. It leads to chronic fatigue, loss of sexual desire (libido), muscle wasting, depression, and bone loss. After a definitive diagnosis is made through blood tests, hormone replacement therapy is administered.

  Penile Curvature (Peyronie's Disease)

It is a condition in which pain and curvature occur in the penis during erection due to the formation of plaques (scar tissue) in the elastic tissue within the penis. This condition can make sexual intercourse painful and impossible. While medication or intralesional injections are used in early stages, surgical correction (such as the Nesbit procedure) is employed in advanced cases where the curvature has progressed and calcified.
