Male Genital Aesthetics
Egemed Andrology Clinic: Male Genital Aesthetics and Penile Surgical Treatments
Anatomical issues related to male reproductive and sexual health can arise due to congenital differences, trauma, the aging process, chronic diseases such as diabetes, or purely personal aesthetic expectations. Today, andrology (men’s health) has evolved beyond being merely a physical treatment; it has become one of the most important medical fields that directly impacts men’s self-confidence, psychosocial well-being, and the quality of their sexual lives within the family.
As the Urology and Andrology Clinic at Egemed Hospitals, we listen to our patients’ expectations within the framework of scientific reality and offer the medical and surgical options best suited to their anatomical structures. Our treatments—including penile lengthening, penile thickening, non-surgical genital aesthetics, correction of penile curvature (Peyronie’s disease), and penile prosthesis (penile implant) surgery—are performed by specialists in their field in accordance with international medical standards, while maintaining the highest level of patient privacy.

Male Genital Aesthetics: Our Surgical Treatment Options
Based on an assessment of general anatomy, skin elasticity, and the patient’s personal expectations, permanent surgical procedures under general anesthesia or non-surgical aesthetic procedures under local anesthesia in a clinical setting can be planned.
Penis Enlargement (Length Increase) Surgery
In male anatomy, a significant portion of the penis remains hidden beneath the skin, attached to the pelvic bone by "suspensory ligaments." In penile lengthening surgery, no artificial material is added to the penis from the outside. The medical procedure involves the controlled surgical release of these suspensory ligaments and the extension of the portion of the penis that remains inside outward. The procedure does not impair penile function; the goal is to achieve an aesthetic lengthening in the visible portion, depending on the patient’s anatomical structure.
Penis Thickening Surgery
The gold standard method for increasing the diameter (thickness) of the penis involves using fat tissue harvested from the patient’s own body (typically the lower abdomen or thigh area) via liposuction (Autologous Fat Transfer). The harvested fat cells are purified through specialized laboratory centrifugation processes and injected homogeneously (evenly) into the subcutaneous tissue. Since the tissue is entirely the patient’s own, there is no risk of an allergic reaction or tissue rejection.
Combined (Enlargement and Thickening) Procedures
In patients seeking an aesthetic and proportionate appearance, the ligament release (lengthening) procedure and the fat injection (thickening) procedure can be successfully performed simultaneously under a single anesthesia session. Additionally, in patients with excessive fat accumulation in the pubic region (pubis) causing the penis to appear buried, the removal of fat tissue from this area (suprapubic liposuction) allows the penis’s actual length to become much more visually apparent.
Non-Surgical Penis Enlargement and Thickening Options
For our patients who do not wish to undergo surgery, are hesitant about general anesthesia, or do not want to spend time in the hospital, there are comfortable, non-surgical medical aesthetic procedures that can be performed in a clinic setting (using only topical anesthetic creams):
Hyaluronic Acid Filler Treatments
This procedure involves injecting hyaluronic acid (HA)-based medical fillers—which are safely used in many fields of medicine and are fully compatible with human tissue—under the skin of the penis using special cannulas. It is generally chosen to increase girth (diameter). The procedure is quite short (approximately 30–40 minutes), and the patient can return to their daily routine the same day. The duration of the fillers’ effects varies on average between 12 and 18 months, depending on the individual’s metabolic rate and the concentration of the molecule used.
PRP (Platelet-Rich Plasma)-Supported Procedures
It involves injecting regenerative plasma—derived from the patient’s own blood and rich in growth factors—into the tissue. Although PRP is not a "volume-enhancing" agent on its own, it can be used in combination with filler or fat injection procedures to enhance the medical quality of the treatment, as it supports tissue regeneration, improves skin quality, and accelerates blood flow in the treated area.
Penile Curvature and Peyronie's Disease
In male anatomy, the condition in which the penis bends at an angle different from normal—to the right, left, up, or down—while erect is called penile curvature. While mild curvatures generally do not impair sexual function, curvatures exceeding 30 degrees can physically make sexual intercourse difficult, cause pain in the partner or the patient, and lead to serious psychological performance anxiety.
What Causes Penile Curvature? Two Key Medical Distinctions
The most important criterion in the medical approach and surgical planning is determining whether the curvature is congenital or acquired (presence of a plaque):
Congenital Penile Curvature
It results from the asymmetric development of the spongy tissues (corpora cavernosa) within the penis or from a short urethra (urinary tract) during fetal development. It is usually noticed during puberty with the first erections. This type of curvature does not cause pain, and no hard mass (plaque) can be felt in the penis. It is simply an anatomical asymmetry, and the only permanent treatment is surgical intervention (plication).
Acquired Curvature (Peyronie's Disease)
It is a progressive disease that typically develops later in life, primarily affecting men over the age of 40. It develops when microtraumas in the outer layer of the penis fail to heal and transform into hard, calcified tissue (plaques). These plaques restrict the penis’s flexibility and cause it to bend toward the plaque during an erection. In advanced stages, the condition is characterized by painful erections and a significant shortening of the penis.
Diagnosis and Treatment Processes for Peyronie's Disease
Diagnosis of the condition begins with a physical examination by a specialist urologist and a color penile Doppler ultrasound performed at our clinic. Treatment is determined based on whether the condition is acute (the painful stage) or chronic (the stage in which the deformity has become established):
Non-Surgical (Medical) Treatment Approaches in the Acute Phase
The first 12–18 months of the disease constitute the “acute phase.” Since the disease has not yet stabilized at this stage, surgical intervention is not performed. To reduce pain and slow the formation of new plaques, methods such as vitamin E and oral medications, intralesional (directly into the plaque) injections, and ESWT (Extracorporeal Shock Wave Therapy), which alleviates pain by increasing blood flow, are preferred.
Surgical Correction Methods in the Chronic Stage
In the "chronic stage," when the pain has completely subsided and the curvature has not progressed for at least 6 months, the only way to correct the deformity is through surgery:
- Plication (Shrinkage) Methods: Special permanent sutures are placed along the longer side of the penis to cause that side to contract, thereby straightening the penis. This method is highly effective for penises of adequate length with curvatures of less than 60 degrees.
- Grafting (Lengthening) Methods: These are performed on patients who have experienced severe shortening and hourglass-shaped narrowing due to plaque. The calcified plaque is excised, and a graft taken from the patient’s own blood vessel (autologous) or from a commercial graft is sutured into the resulting space to lengthen and straighten the penis.
If a patient has persistent erectile dysfunction that does not respond to medication, in addition to severe penile curvature (Peyronie’s disease), correcting the curvature alone will not restore sexual function. In such cases, or in patients who have completely lost their ability to achieve an erection due to conditions such as chronic diabetes or prostate surgery, the permanent and definitive solution is penile prosthesis (penile implant) surgery.
One-Piece (Malleable) Prosthetics
These are silicone cylinders with a flexible design. They maintain a rigid shape at all times, but thanks to their internal wire mechanism, they can be bent downward to be concealed inside underwear when not in use. They are very easy to use, pose no risk of mechanical failure, and are more cost-effective.
Three-Piece (Inflatable) Prostheses
It is the gold standard method that exactly mimics the natural erection mechanism. It works with a concealed pump mechanism placed in the scrotum. The patient presses the pump before intercourse to inflate the cylinders, and after intercourse, deflates them to return the penis to its completely natural (flaccid) state.
Important Information: Penile implants are placed entirely inside the body and are not visible from the outside. The implantation of a penile implant does not in any way affect the patient’s sexual pleasure, ability to experience orgasm, or ability to father a child (ejaculation) .
Whether it’s genital aesthetics, correction of curvature, or prosthetic implantation, the medical process for all procedures requiring anesthesia is meticulously managed at our hospital:
- Medical Examination and Consent: Prior to the procedure, our urologist assesses anatomical suitability. The possible outcomes are transparently discussed with the patient, and informed consent is obtained.
- Hospital Stay: Surgeries typically last between 1 and 2.5 hours, depending on the technique used. To ensure early infection control and patient comfort, we generally schedule a one-night hospital stay for our patients.
- Recovery and Restrictions: Patients can return to desk work or a light daily routine 3 to 5 days after discharge. However, to ensure full tissue healing and proper integration of the prosthesis into the body, it is medically necessary to avoid strenuous physical activity and sexual activity for the period determined by the physician (on average, 4–6 weeks).
